Unfortunately I can't help with where to get it, but the answers to your questions is apparently available from somewhere:

What you're looking at is a split drop bar, which can take twist shifters(apparently!) up on the flat section. It might have been horribly creaking or something while riding, but given how often the flats to drops question comes up it ought to be helpful to some.
