Mysterious flats

Over the past year I've gotten 4 or 5 flats. They have all occurred inside the house, and none have been due to tire punctures. The most confusing part is that each puncture has been located on the _inside_ diameter of the tube, in contact with the rim tape. The punctures are not happening over the eyeholes, and they happen anywhere along the tube.

I've checked and found no culprits in the rim, eyeholes, rim tape, or inside of the tire.

The tube and tire are always seated properly, and I moved from simply replacing the rim tape to using two strips, which seems to have mildly subdued whatever the issue is.

If anyone more knowledgeable could offer some insight into this issue I'd really appreciate it.

EDIT: I'm using a cxp33 rim, vittoria action-hsd tire, and Giant tubes.

Are the punctures occurring in the same location on tube, like 3 o'clock position with respect to the valve stem?

AH, the placement would've been good to mention. No, they are not occurring in the same spot.

Are you putting the tire on with a lever?

I use two plastic levers when removing/installing the tire. The punctures have never occurred where I've used the levers, if that was possibly your next question.

That's your problem. You pinch the tube, but the puncture takes a while to work through. Don't use a tool to install tires.

Oh, sorry I missed that. Then I dunno.
