Bottom Bracket came loose?

Me and a friend were doing some tuning on his bike, and then all of a suddon the the crank froze up. We noticed the bottom bracket was comming loose from the frame. We managed to tighten it up a bit so we could get it to pedal again. How would this come loose? and what tools are needed to tighten it back up?

seized bearings maybe. you will need a isis crank puller and most likely a 8 notch bb tool. looks like a truvativ isis bb and crank
