adjusting RD - fine by hand, can't rear largest cog when assembled

Had to replace a hood on the shift handle, so I disconnecting a beutifully adjusted Shimano Dura-Ace RD. Upon reassembly, I can't get it to shift into the largest cog. If I pull the cable at the downtube by hand the RD moves to the right spot under the largest cog. When I try to tighten the cable the RD always relaxes down one gear. I tried the barrel adjuster but that didn't do it. Any advice?

You should be able: 1) Set shifter for smallest cog (i.e., for the highest gear), 2) Pull slack from the cable, 3) Tighten the cable, 4) Use barrel adjuster to remove any further slack.

Are you doing the above?
