Stem length
 
When a stem is listed as 40d 110mm I get that it means 40 degrees off horizontal and the 110mm is the length, but exactly where is the length measured? Along the angle of the neck or center to center horizontally?

Correct. Stem lengths are measured along the extension from the center of the steerer bolt to the center of the handle bar clamp. The actual horizontal projection will be less.
