Shifter not relaesing cable, could cable head be problem
 
I installed new cable and housing last night and now I am getting unrelaible shifting from rear derailleur. I get things dialed in and everything is fine for one or two trips through the gear range, then suddenly the cable will stop releasing when I down shift so chain won't move from larger cogs to smaller. The cable seems to be sliding freely through the housing. One issue I am wondering about is the cable itself. I used shimano cables (Ultegra 6500 9 speed shifter and derailleur) which have a larger head than the cable I replaced. I thought the shimano cable would work despite the headsize difference and the head seemed to seat okay inside the shifter, but might it be possible that the larger head is not seating correctly and therefore causing the shifting issues?

Shift all the way down to the smallest cog and take a look? It's definitely possible it's not seated properly. Also check for kinks in your housing, the loop from the chainstay to the RD is notorious for kinking right at the end of the cable ferrules.

If you have Campy Ergo brifters, the shift cable end fitting must be smaller then that for a Shimano STI. Using a Shimano cable will either not let the head seat properly or, worse, it will jam firmly in the brifter and be very difficult to remove.

Take a look at your cable housing ends and make sure that they are cut square and that no small strands of reinforcing wire are sticking out, which could jam the wire in a socket or ferrule and cause the wire to hang up. Also make certain that the ends are properly seated; the ends of the small loop of cable entering the derailleur can easily get cocked askew if that loop is too long or too short, which can jam the cable. You could also use a file and carefully reduce the size of the cable head to match what was installed before.
