I heard back from HED and here's the answer...drum roll please:
- the rear drive is a Shimano Ultegra FH
- the non-drive is a 6001 cartridge bearing

Any recommendations on bearing MFRs for the 6001? Right now, I'm looking at a Phil Wood that I can get for $5.50. Any issues there? I'm not interested in ceramic or a ceramic hybrid, I think good ole steel will be more than sufficient. Thanks again for all the help.

Any good-size town will have a bearing supply store that has them in stock or can get them cheap. As an alternative, ask at an auto parts place. Cartridge bearings are often used in alternators. 6001 is a very common size.
