Cable wonít stay on friction shift derallieur

When I attempt to shift on the friction shifter, the cable just slips. Any idea why?

Where is the cable slipping? At the pinch bolt on the derailleur?

Does the derailleur easily move by hand without the cable?

Odd

You are getting it tight, right? No stripped threads?

Also, there is a washer under the pinch bolt? Is it bent? Often the washer is notched in a way to retain the cable, or there is a groove so the cable won't slide out from under the pinch bolt.

Photos can often help.
