I'm just shy of 6'-3", with a 34" inseam. My best fit is on a 61cm Gazelle CM. I'm running a setback seat post, 120mm (I'm fairly certain) stem, and it fits like a glove. This is set up to ride on the hoods with ergos. If I were riding the drops, I could probably shorten the stem a bit. I have a little saddle to stem drop too. You need to factor the difference in saddle/stem height and hand location into your measurements.

If it doesn't have to be vintage, Surly is all over the "long top tube" thing. I've got friends who love them, but there's no way I'd be able to set one up for me that didn't look silly.

Thank you, I did try some Surly's last week and there was one that seemed to fit nicely but i just don't love the frames, they are aggressively boring haha. I would probably go for the Masi Strata Vita that I liked, or maybe the Specialized Sirrus Sport. But really I want a cool vintage frame, but I'm starting to think maybe it's not possible. ;-(
