How does the quill system work?

Hello! I am new here and I am very desperate to understand something for my custom chopper build. I am using a threaded fork and threaded headset, and I want to install a quil stem. But I am not sure how it works... Do I need a star nut? If its something else how does it look and where do I get it? What part of the bike do I put that mysterious part into?

You usually need a 6 mm allen wrench to tighten the stem's wedge or expander against the steerer.
