Age- and use-hardened pads are by far the biggest culprit in terms of generating shards from perfectly good rims and then galling the rim's surface.

Most DiaCompe road pads seem to be immune from hardening, I'm still using original pads on 1970's centerpull and canti brakes from this company and my Superbe pads (made by DiaCompe) are also still in the game and performing smoothly without generating any shards.

Sanding the surface of hardened pads is unfortunately a short-lived victory it seems, but at least it can be done very quickly by forcing the wheel to turn through an applied brake with coarse beltsander stock moving along with the wheel:
