I've got one (with the pin-up) on my track bike; very stiff stem and I too get a lot of compliments on it:

Not to sound like a lech (( try to keep it a secret!), but where does one get the pin-up inserts? For a friend, you understand.....

I see them on ebay from time to time, do a search on Alter, usually comes up with a few bodies
and pin up covers.

i think the idea of the pin-up inserts is hilarious, I mean it basically puts it in the perfect spot to look at when you are on the bike. I am surprised an idea like this was ever able to make it into production with such a large and prestigious company like cinelli haha

Before: $25 impulse buy from eBay

After spending a lot of money and having a good time doing it:

Before: impulse $10 buy from eBay

After a new paintjob and a thorough cleaning of the bike:

Front fender has a bad dent so I left them off. Rear brake has been added. Still waiting on new wheelset.

Before: JC Penney's finest

After some good cleaning and new paintjob

well, a new saddle, pedals and handgrips from Wal-mart.

Before:

After: so far

a fellow alleycat racer has expressed an interest in this bike. I just might let it go.

I posted this one in the COTD thread, but I have a nice set of "before and after" pic so I thought I'd toss em in here, too. A late 80's Centurion Cavaletto in disrepair, that I converted back to a road machine with a triple drivetrain. It came out very well:
