As you may note from the chart just posted, their sizing is a tad smaller than most metric-sized shoes, at least at the larger end of the scale. I wear a US 11, and in most cycling shoes, a straight-up 45 feels roomy. I relied on their chart and bought my MX-81s in size 45, but found them to be slightly smaller than other brands. Fortunately, they come with fairly thick insoles, and I found that just replacing them with a thinner set gave me a perfect fit.

I've owned several pair of Lake cycling shoes over the years and always found them to be the best fitting and most consistent with standard American shoe sizing.
My favorite was that awful looking purple pair that I got from Nashbar on clearance a few years ago....they were the only pair I actually "wore out".....

actually lake is running .5 Euro size small. GCRyder is spot on...they run a little small
for instance. I wear 42. A lake 42 squeezes my feet. Lake 43 is much better.
IMHO go 1 euro size higher, or at least a half-size if you have the option
