Personally I've been lucky as I've only had a broken chain (4), and numerous flat tires (3). All subjective, of course.
Agree. When commuting, everything gets bagged securely. Clothes get rolled-up, soiled undies get their own Ziplock hamper bag, etc.

My two worst commute disasters(level 9) was being struck by cars....both by women on cellphones. But, I played the insurance claim game to the hilt, and was awarded close to $30,000 for those incidents(no lawyer!).

Not a disaster, just a stupid mistake, but...

Rode to work. Locked up the bike at the rack. About halfway through the day I realized that I had borrowed my wife's cable lock and neglected to bring the key with me. That's right--I did the bicycle equivalent of locking my keys in the car.

Luckily, I keep a folding bike in my office for getting around campus on the days I drive in. Not the best option for the ride home, but any port in a storm. So I grabbed it, unfolded it... aaaaaand it had somehow developed a flat tire overnight.

Wound up Ubering home.
