What is the best or accepted way to handle this kind of thing? Send them a message, ask the moderators for help? I haven't experienced such a recurring forum error like this, but it should be corrected if possible.

I think if you look at the number of posts and the number of times a mistake is made like this, you will find the mistakes are pretty rare. Probably no need for moderators to get involved.

Just send a polite private message to the responsible party, fix what you need to, and move on. No need to point fingers and raise a big stink.
