I wear safety glasses almost 100% of the time, I have only one usable eye and want to protect it.
If riding at night I wear a reflective vest

I have a plastic (like Rubbermaid only generic) tub zip tied onto my rack, it acts as both a storage compartment and a bumper.

Very bright 600 lumen front headlight, usually on strobe, but sometimes on full high, a PB SuperFlash in the back, a helmet, a ding-ding bell (to satisfy the law, it doesn't do squat.) A helmet, fenders, reflective clothes. It is always light in New York City, so I don't worry too much about being seen.

My seemingly unique contribution to the above listings of safety gear is that I use both left and right rearview mirrors, in my case Take-a-Look eyeglass mounted ones. I got the idea from a cycling companion who used only a right hand mirror. The right hand mirror affords a pretty good rearward view, but is particularly useful:

• Riding on the left-hand side of a one-way street
• Riding in the middle or left lanes of a two-way thoroughfare
• In a rotary
• On a curved road to the right
• When the sun is directly behind, usually one mirror can be positioned away from the glare of the sun.

I use tires with reflective sidewalls. They light up when headlights throw a beam my way.

I'd also recommend reflectors. They do add visibility, especially those on moving parts (pedals or crank arms).

Also, reflectors are required by law in my state, and most likely in yours as well. You don't want to give a driver an easy way out — "No reflectors! Couldn't see him!" — should you end up in a conversation with law enforcement.

Reflectors are easy to come by. Just ask a bike shop or a bike co-op, if you have one. The co-op in my town had drawers full of reflectors, as most riders strip them from their bikes first thing.

Nice! I sometimes carry my CZ-70 in the summer (Firestar .40 in the winter sometimes). My girlfriend has CZ 82, it's one of my favorites.

The Airzoundz can be incredibly satisfying. I also feel like it serves to shame people. Honking a horn seems to make people wonder what the vehicle getting honked at did wrong, rather than sometimes people thinking you are crazy or aggressive or something.

hi-vis jacket/shirts...I have quite an assortment from Alert Shirt so have one no matter the weather
gloves with reflective detail on them (though I've found the white/black hi-contrast gloves seem better)
my Keens have a protective toebox. For when I run over my own foot.

All of my panniers/trunk bags have reflective piping, and they all have little cheap red blinkies attached. (whenever I get a free cheap blinkie from some running/cycling promotion at work or the LBS I just hook it on a bag.)
There are reflectors on my pedals that came stock, but since I can barely see the pedal reflectors of other cyclists, I assume they're pretty useless.

No wheel reflectors, because the ones that came stock were such cheap pieces of excrement they were shattered at the bottom of the box when my bicycle arrived.

I work third shift, 11PM-7 AM.
On my body I wear:

a vintage Bell Tourlie helmet, with a Coleman 100 lumen (claimed) headband light. Extra reflective bits on the helmet.
A Take A Look mirror, also mounted on the helmet.
Prescription ANSI Z-87 safety glasses. Yellow transition lenses, they turn brownish in UV light.
Dork bands on my ankles.
Padded gloves.
A J&G rain jacket, bright yellow w/(what was once a lighted) reflective vest.

The bike wears:

B&M Lumotec IQ fly+, powered by a sidewall dynamo.
Front/rear reflectors mounted on the vintage Cannondale bags.
Winkku turn signal/running lights/mirrors w/reflective tape on the frontside of them.
Frame mounted reflectors.
The back of my saddle is reflective.
Vintage long wheel reflectors, amber in front, red in rear.
Down Low Glow or "Ghetto" version of same. The "Ghetto" version consists of two Larry lights w/amber taillight tape on the lenses, zip tied to the frame.
Pedal reflectors. I used to have pedal with LEDs on them, but I couldn't use toe clips with them.
Planet Bike super flash mounted on rear fender. Usually in flash mode.
Two generic blinkies set to solid on, mounted on the V-racks of my Freeradical. Extra reflective tape on the V-racks as well.
I used to rock a vintage Canadian mfg. brake light, but the switch died, would like to get it repaired.
I am also planning to add a 4 led tail light. I just need to figure out the mount.
Red reflector on the bottom of my Freeradical.
I also have a safety Gadsden DTOM flag, reflective tape on the shaft. I would like to get a set of the helmet vent lights to mount at the top of the flag shaft.

couldn't help but notice...some of you mess with Air Zounds? Between lights, bags, locks, clothing etc. I can't imagine messing with one of those as well just so I could essentially 'yell' at somebody.
