What about a recumbent? Albeit, they could be more expensive but from both comfort and efficient-pedalling perspectives, it could be your answer.

About seat height. My neck doesn't do well if I go lower.

If you're looking into bullhorns, I'd advise getting one with a longer semi-vertical at the end of the bar. I can't quite get my entire hand on these (Nitto RB-002's), and I wish I could.
