It's very counter-intuitive, but I think it (pretty much) stays the same. The medicine ball and the basketball fall at the same rate:



Of course, the medicine ball has more mass and gets a bigger pull from gravity, but it also has more inertia, so it takes a bigger force to get it moving.

Imagine you and I are both riding our Scwhinn Stingrays, and we both have the same pedaling strength, pound for pound, even though I weigh much more than you. We would take off at the same rate. I would have the flywheel effect on my side, meaning I might be able to run through a brick wall where you might bounce off. But, the acceleration should be the same.

If you want to get into aerodynamics, I guess the answer could change slightly. Assuming fat is less dense than muscle, maybe she has a bit more air resistance per pound than him, so the cart might accelerate very slightly when she falls off. But, her rounder shape might make her a bit more aero, too, so I am less confident on this part.



Their shapes would both fall in between, but hers might be closer to the figure on the right, while his is a bit closer to the figure on the left. Strictly a guess, but I think the aero shape would be the critical difference, meaning the cart might slow down just a bit when she came off.
