I keep the basics in a seat wedge: tube, patch kit, levers, multi-tool. I also keep a photo copy of my drivers license and health insurance card in there. On a short/solo ride I might throw a cell phone and keys in there too. For longer rides I'll wear a jersey and I'll put some money, real ID, phone keys and maybe food in the jersey pockets, along with a map/cue sheet for group rides.

Personally, I never get tired of seeing Khuon's what-I-carry post. As for me, on last week's shop ride, I just left the wedge off and carried the following in my jersey pockets:

I have a very cool "over one shoulder" sling-style/back-pack which always has a couple of tissues in it, my ID (emergency numbers, blood type, drug allergies, insurance info., etc. which I typed up on my computer and printed out onto a piece of paper), some chapstick, a flat billfold with $1-$5 in it at all times, my keys, and the best part is on the should strap, at about "breast level" is a little pouch for my cell phone. Sometimes I return DVDs or videos at a local Blockbuster, so I might have one of those in the pack, too. Mostly, I like to ride "bare" - just keys which I "hook" onto the cables on my handle bar and which jingle when I ride, and my water bottle. Keep all the other junk at home. And if I were to have a major breakdown, I ride within a certain radius, I'd just have to walk home, so no need for repair stuff - plus I am careful what I ride over and on the look out for nails, glass, etc.

I Kuon's vein: I like to bring a spare frame and a couple of wheels as well as some other odds and ends like a spare saddle and handle bars with me. I get SWMBO to ride it so it saves me the trouble of carrying anything, if something breaks on my bike, I use those spares as needed, she walks from there on with the rest on her back. If she complains too much I show her these photos

I usually only go on short rides (15-25km) so i don't really carry that much. I have a water bottle on the frame, and in my bag that i wear on my back i have a 1.5L water bottle, wallet, keys, cell phone, and my mp3 player. I think i'm going to start bringing a protein bar with me from now on.
I guess if i was going for really long rides i would have more stuff with me, but then again, i don't really own any of the stuff that everyone else mentioned.

I carry a patch kit, co2 thing, tire tools (can't call 'em irons they're plastic) and wrenches in a seat wedge bag, but I'm considering just keeping that stuff in my messenger bag since I carry that everywhere.

I carry on every ride the following: 2 differant sized spare tubes - one is for a 20" wheel the second is for a 26" wheel, a first aid kit, baby wipes - to help clean road rash should I have a crash & there isn't always toilet paper or a place to clean my hands when I use the bathroom, cell phone, multitool, C02 "gun" with 6 cartridges, patch kit, emergency chain link, bicycle tire pressure gauge, liscense, atm card, League of American Bicyclists mebership card, medical insurance card, money if I have any to carry - usually no more then $10.00 or so, handkerchief, MP3 player - programmed with about 27 of some of my favorite tunes, 2 small battery operated external stereo speakers, 4 extra AA batteries, 1 extra AAA battery, Camel Bak Classic 70 oz. water pack. The water pack is attached through the seat straps everything else is in a Vision Day Bag & attaches to the seat frame covering up the Camel Bak.
On long rides of more then 50 miles I carry all of the above plus the following: my prescription Flonase, small bottle of pain reliever - such as Tylenol & Advil, etc and pepto bismal chewables

I use a small Tool Bottle which holds Mobile, Surgical Gloves, ID, 130 yen, 3 Tyre Levers, 1 Tube. Pump on frame.

I carry all the following stuff in a seat because I ride into remote areas where getting help would be impossible since most of the time there is no cell coverage, and walking could take hours, and because I ride an older bike. If I take my Camelback Rogue I only put Power Bars in the pockets and nothing heavy due to not wanting anything heavy on my back. Most of the time of real long rides I carry a handlebar bag with food and a jacket.
In the seat bag I carry a ultralight folding tire, 1 ultralight tube in its box, 6 glueless patches, 1 tire boot, Park MTB3 mini tool, Eddie Baur (cheap Leatherman copy from Target) very small folding pliers, tire irons, Quik Stik (rapidly removes tire beads), VAR (puts on the last inch of a tire in a snap without any possiblity of damaging the rim or tire or pinching the tube-though now that I'm no longer need and using Armadillo tires I may stop carry this), 2 AAA and AA bats for my front and rear flashers, button bats for the computer, $25 cash, phone card, ID with blood type and phone numbers, and a small first aid kit for minor stuff and headache pills, and sometimes a couple Power Bars depending on how far I'm going.

All in a small plastic bag in a jersey pocket. No ugly seat bag necessary. That's all I ever carry, and I commute 4000+ miles a year.

In wedge I always have a MultiTool (Crank 10), small patch kit, tube, tire levers, small tube of sunblock, and an old wiping rag. I stick my wallet along with the house and car key in one of those little topbar pouchs because I'm nervous about sticking them in my jersey, despite the fact that I don't see any way they could fall out. I usually have a disposible camera in a 99 cent wallmart camera bag attached to my stem. That way I can grab it with one hand while riding, snap a quick pic, and slip it back. Oh ya, always a minimum of one large water bottle. I got hit hard with the heat many years ago in the army, and I've always had to be real proactive in keeping my fluid levels up.
