stem shifters.
I've been really annoyed about not being able to find a minimalist setup for my current build.
So I've been wanted these bars and brakes, but the bars don't take bar end shifters. Others tend to have too far a reach, and tend to be mainly for TT use.
Finally found that Paul Engineering do thumb bracket adaptors.
Even though they were snubbed and only put on cheap bicycles, wouldn't it be nice for a revival on thread-less stems?

Custom Frame Builders have made steel stems and fitted the bosses for downtube shifters and a near by stop onto the stem for cables .
looking at the bikes displayed on some of the hand made bike shows that have taken place around in several Regions and cities can give you examples
there have been bikes sold in the past with stem shifters
Some of my X-GF's preferred them, bikes in the 70s offered them .. hit the bins and basements of old bike shops
dont expect indexing that far back.
Threadless steerers you can have someone fabricate a spacer with the shift boss and housing stop on it
and have it include a pinch bolt to keep it from rotating when you push/pull the lever.
open that Wallet and much can be done

zigpy, I cannot tell from your profile if you are male or female. If you are male, there is a very good reason to pass on stem shifters. Think about it; if you hit something like the side of a car and stop really fast, what part of your anatomy is likely to get way too close to those shifters.
In my bike shop days, we encouraged those we believed might listen to upgrade to either downtube or bar end shifters. At my last bike race in a small city in New Hampshire, I stayed at an old hotel, then mostly housing live-ins. As I was hauling in my bike stuff, an older gentleman approached. He wanted to tell me that he had been castrated by those shifters.
Use them at your own risk.

Quill stems and threaded headsets? I'll never order a custom frame again with anything else. I love the complete ease of adjusting stem height, that I can do it on a whim mid-ride or just before I start and know I will not to have to stop to readjust anything (assuming I got the bars straight, an issue with any system). On one bike I have two complete "cockpits"; bars, stem, levers, cables and brake calipers. It take less than 5 minutes to completely change the bike.
Besides, good quill stems look so much better. Cinelli 1A, classic TTT, Nitto Pearl.
