Adapter info..

I have a 04 trek 4300 non disc bike. I recently purchased a trek part # 210648 rear disc frame adapter. It mounted perfectly, but it's a flat mount. I have a set of Sram level t disc brakes with a 160mm rotor I wish to use. I'm confused as to which adapter I need now to add to go from the trek flat frame 210648 adapter to the post mount brake. An IS to post ?
Before you ask, yes I have a disc wheelset, and I'm just trying to give my nephew a decent bike set up.
