Need info re: disc brake on a Schwinn Homegrown frame

Hi, I'm kind of new here, and I need help: I have bought a Schwinn Homegrown, v-brake equipped bike, and I am pondering fitting disc brakes to it. In the front the Rock shox fork is prepared for this, but in the rear, the frame has this plate, with 2 threaded holes. I can not undertand the setup. Is there some kind of an adaptor needed, is there a specific caliper that needs to be used?
Thanks for any insight.

Thank you! I am slowly going forward. The frame is a 15", and I am wondering about clearance, but I think I am on the right path.

from that above, will the "AD- PMR" work with the Hope Mini rears w/160 mm disc and International Standard mounting??? I know this is an old post, but if you're out there and know, please clue me in. I have a 99' Homegrown.

Thanks!
