How to accurately measure shoulder width?

Notices
Road Cycling “It is by riding a bicycle that you learn the contours of a country best, since you have to sweat up the hills and coast down them. Thus you remember them as they actually are, while in a motor car only a high hill impresses you, and you have no such accurate remembrance of country you have driven through as you gain by riding a bicycle.” -- Ernest Hemingway

How to accurately measure shoulder width?

I've seen many people say to measure them but nowhere have I seen "how to measure them". The closest I've seen was someone said to measure to the centers of the shoulder, which made no sense at all. I really don't want to spend the money and time by experimenting with a few different widths of bars. Is there a more "scientific" method, maybe a link to a site to get a starting point?

I also know people that insist that the correct way to be fitted for bars is to pull them across your chest and under your arms and they should just clear. On my newest bike I used that method and it is the first bike that I have ever used 46's. They seem to help in the drops and have virtually gotten rid of any hand fatigue on the hoods.
