*Raises hand*..................I don't understand how someone can fall. It only makes sense to unclip before you stop.

It did bleed pretty bad. I took my sock off and laid it over the cut and wrapped my spare tube around to keep it from bleeding too much as I peddled to the Dr's office. That was the worst part of the whole thing, the tube was so tight it kept my thy muscles from being able to move freely.

I fell over side ways and dislocated my shoulder trying to learn how to kick out at a stop light. No scars but I think I got the TDS* award

I bet UMD has his releases pretty tight, but the rest of you who have never fallen, do you have very loose releases? If you loosen the thing all the way you reduce the risk of not being able to clip out since any motion of your foot will pop the release, but then you run the very real risk of unclipping during serious exertion, like that last bit of the really tough hill, not fun.
Do you clip out 30 feet before you think of stopping? Probably means you never have a 0mph crash, but it also means that you are finishing your ride much later than I am since I will come up to that intersection, track stand until I'm sure there is a safe crossing, and then go. Of course for me (still working on my skilz), that does mean that I sometimes loose my balance and have to clip out at the last second. And yes, I was less than successful once.
Also had a 0mph on my first ride on my new bike. The releases were tightened down as hard as they could go to provide feedback in the fit process, but I never loosened them. I remembered to do that the hard way. By landing on my hip in front of 7 strangers and a friend.
So, 0mph crashes = 2. Number I am embarrassed about = 2. Amount I honestly give a s**t about it = 0.
All that said - to the OP - am I crazy or does it look in the after like they stiched you up but didn't clean around the wound? Didn't stare too hard, but seemed odd to me.

Ever captain a tandem where the safety of two riders is in your hands, and the stoker doesn't unclip at stops? Try it along with some mountain biking, makes riding a single roadbike a snap!
No need for loose tension on the pedals. Unclip 1 foot 2 or 3 yards before the stop, and the ability to trackstand for a few seconds makes lights and stop signs a breeze!
4,000- 7,000 miles a year on the bike for the last 15 years, stopping is second nature.
Uhh nope never had a foot come out on a tough ride up the hill, err I mean mountain!

Unlike the other cycling gods on this board who can do no wrong, I feel your pain brother.
Here is my Cat 5 "sharkbite" I got a little over a year ago when I first started and went clipless. Knock on wood, that was my first and last fall. The scar's still very visible. The worst is behind you, it only gets easier from here.
