Lennard Zinn in his column in Velo News said that they put pedal spacers on about 40% of the bikes they fit.

WOW! Do you use spacers?

How do you determine if you need spacers? Have someone observe (or videotape) your knees while you ride on a trainer and look for even spacing between the knees and the top bar?

During my fitting, he had to move the cleats all the way to the inside of my shoes to get everything lining up the way it should. That combined with my natural tendency to pronate resulted in my foot over rotating to the outside, so a couple of washers moved the pedals out a bit and let him move my cleats back towards the center. I wasn't having joint pain before, but I'm also not having it now and it does feel better, particularly when I stand while climbing or doing intervals.

I was having knee pain, read about sitting on a counter or something high and let me feet dangle. Both feet were angled, with the left noticeably more so. Added the cleat wedges and the pain has been gone since
