Worst cycling item you've had/have.
The point of this thread isn't to point out that some brand sucks or some are better but just to show something you have or once had that was crap.
I will start with my
ACE-18 Alexrims, they must weight a ton, they bend and go out of true for about no reason.

Back when I used a seatpost bag: specialized mini wedge. After about a month of use the zipper started unzipping itself so it would dump my keys, tube, cell phone and co2 cartridge on the road while riding. This was probably a good thing because I started using my pockets and look more pro.

those "lightweight" titanium skewers that use an allen key instead of a quick release. they weighed an awesomely light 23grams and I installed them to manufacturer's torque recommendations but still managed to strip the bolt on the rear skewer.

I have these on my cyclocross and absolutely love them. Yes, they weigh a bit more but they are indestructible. Every time I hit a pothole or something, I swear I have ruined them...after 12,000 miles, they haven't changed one bit. Haven't had to true them yet and don't need to. Awesome wheels.

Luckily, shortly after joining BF you became a freshman in college and instantly learned everything there is to know in the world.
For me:
A selle italia xo saddle...near instant numbness on rides. Terrible.

Mine would be the original Cinelli clipless pedals, circa 1984. They had two rails that you slid the cleat forward to enter. To lock in, you reached down and pulled out a tab that caused a small rod to raise up into the cleat. There were two ways out:
1. Reach down and push the tab in to lower the rod and release the cleat
2. Fall down. The ground will push the tab in, thus effecting release.
As bad a design as that is, the reason I stopped using them was that the spindle was a bit undersized. Several early adopters were gloriously scarred when they snapped them off. I still have a set in my spares box.
