Convert stem type or get adjustable?

I have a Trek 1000 with a quil stem. In making the bike fit my body properly, I needed to raise the stem up an inch or so. However, I was maxed out at that, so I needed a longer stem. Nothing I could find at the used bike store was longer. Also, is the skinnier type (supposedly 1 inch, but I measured the tube at 6/8".

Options:
Upgrade to Ahead setup. Get the vertical post insert going into the fork, then the ahead stem.

OR

Get old school adjustable quil stem so I can angle it wherever I need it to be and those are pretty long so I'd hope it's high enough.

that would gain me 4 cm for sure (forgot to mention mine is 15 cm long). what if I wanted to change the angle though? that's a 0 degree rise right?

I'm facing a similar set of thoughts as I carry my arm around in a sling following surgery for a busted clavicle...don't think that 120mm reach will be do-able for a while. I'm thinking about the extra rise of that Nitto. I looked at an adjustable, but reckon the constantly changing reach along with angle would make steering really freaky. I've also looked at a threadless adapter, but dont' think it'll look right on an older bike...I've got a week or two to decide.

+1 for Nitto Technomic stem as per post #2. You can probably find one NOS or gently used on ebay.

I'd prefer a situation where I can adjust angles since I won't know how that nitto feels until it's on (and paid for) and it might be uncomfortable. LBS guy said I could swap out for a longer threaded fork and attach the adapter to that, then put the Ahead stem on it. Right now my forks steering tube is 133mm, and the frame part is 113mm, so only 20mm sticks out. Maybe I can get one where 50mm sticks out and i can put the adapter (first pic) on that.

the 90/65mm is my original stem below and above the minimum insertion line, not to scale obviously
