I make a conscious decision to put pressure on the outside of my left foot on the downstroke but I often experience foot pain along the 5th metatarsus at the end of a race.
I don't experience knee pain yet but my left calf does tend to cramp/tighten more than the right so I assume knee pain is not too far off.

If all's fitting well, you shouldn't have to think consciously about where you're applying pressure with your feet.

I think the Specialized BG shoes are terrific.
I think some orthotics are terrific and should be considered by everyone.
I think custom orthotics where they take a cast of your foot are garbage. If your foot is in absolutely perfect form, then custom orthotics are good but a waste of money. If your foot isn't the perfect form, then custom orthotics will not improve your foot shape but will keep it from getting worse. Your foot is suppose to have a certain arch. If your foot doesn't have this shape, to make orthotics using a mold of your foot will not improve your foot, only not make it get worse. An orthotic which is designed to be what your foot is suppose to look like, will reshape your foot to what it's suppose to be like. This is what orthotics have done for my foot, reshaping into what it is suppose to be like.
