i think its generally used in a lighthearted if not, slightly ribbing kind of way. i use it all the time when some dude in his full kit passes me at breakneck speed on the bike path...like "go you lance"

When I was out on the ol Mup a few months ago with my wife and kids, we were a approached double paceline. The guy in the front yelled, stay on your side to my daughter, she yelled back, you stay in yours, Lance!!!! I'm so proud of her....

I'm female so I've yet to be called Lance, but some teenagers were "heckling" a male cyclist passing them on the street once. I was following a few yards behind but they didn't say anything when I passed by, so I yelled out "What? No love for me???"
They laughed, but I think their intentions were to get the guy kind of worked up...

The fact that anyone says anything from a car makes me feel somewhat safer. At least they notice me and are less likely to run me over, unlike someone texting or on a cellphone.

I was out riding one day during my chemo regimen. I'm this pale, hairless rat riding an old mountain bike really slowly, and feeling like if the chemo didn't kill me, the ride would. A couple guys passed me and I heard one say to the other (loud enough that I believe he wanted me to hear it) "Hey look. Lance shaves his legs!". I just smiled. In my case, it was most definitely an insult. I don't care. It was a beautiful New Year's day, I was outside, riding and every heartbeat felt like my heart was going to pound right out of my chest - reminding me I still had a pulse. I feel bad for the guy who said it, though. It must suck to be that insecure to have to make comments like that.

My friends from work (all non-cyclists) used to call me Lance. They were obviously trying to make fun of me, but I told them I took it as a major compliment. After that... they stopped calling me that. LOL A couple of months ago 3 cyclists in full kit, etc... were stopped drinking water. As I was passing by one of them says, "Watch out, here comes Lance." Don't know what brought that on, but I'm assuming they were calling me a poseur/Fred since I was in just regular shorts and t-shirt. Who knows... didn't really care what they thought.

It's just something non cyclists say to us. Lance is the only cyclist most of them know about so that's the name they throw out. Makes no difference to me what they mean by it though I'm sure some are just having fun and mean no harm. Best to take it that way if you can at any rate.

If all they said was "Hey! Lance", it could be a compliment or an insult, depending on how you interpreted it. In any case, respond with "Hey! How's it going?" It gives them nothing to come back with.
