Not riding enough. And yes, trying to stay relaxed and not tighten up my shoulders gets me too, but only on one of my bikes (working on getting a better fit dialed in - it's getting there).

It happens all the time to me and I won't play roulette with shooting intersections.
I was riding the other day with a fast guy and we were taking turns pulling...and we were flying for a few miles. We came up on an intersection that he just didn't see and I yelled out just in time as he was going to go barreling through the intersection. He locked up his brakes and just missed traffic.
But it is generally worse than that. Guys know the intersection is there and time cars...not leisurely...but aggressively. Not my idea of a good time but it is sport for many I see out on the road.

I tend to lock my elbows when I get tired. Then my hands go numb and my shoulders ache. That and pushing my toes up to far in the toe box of my shoes so they go numb too.

I just can't do a hard ride, solo. The second I see a bird, or a squirrel, or a tree, or a blade of grass I get distracted and next thing I know I'm on the 42x25 going 12 mph. If I'm doing an interval and have a specific goal, I have no problem doing it. But just going out an hammering... I can't. This goes into my next problem... Every solo ride winds up being a recovery ride.

I almost never eat on my rides less than 60 miles, and I often end up paying for it on the homestretch. Especially considering I almost never eat breakfast either...

Turning almost every training ride into intervals. It comes from my background in swimming: interval training is just what I'm used to. I need to constantly remind myself to diesel it a bit more.

Down shifting is not being lazy, what gives you that idea? Climbing too slowly is being lazy, but downshifting and spinning fast to keep up power output and speed is smart and effective. Mashing is bad for your knees.

Turning my head side to side more to relax my neck.

Change hand position much more often.
