Foot pain/swelling possible cleat issue?

Long story short I moved my cleats way back. A lot of good came out of this but one of my first rides after doing so, something unusual happened. I woke up the next morning with sore and swollen right foot. Just behind the toes about a 2 1/2 inch band. It wasn't real bad, kept me off the bike for a couple days and then no more problems...until yesterday.

I put some new cleats on some old shoes (mtb shoes so I don't have to fight the SPD cleats on my road shoes) and went for a ride. All went well and I enjoyed being able to "find" my cleats easily. This morning I woke up in pain. Foot swollen pretty good. Same foot, same spot. Can't get it into a shoe or walk. This time when I walk there is pain on the bottom of the foot just in front of the ball. ZERO pain or discomfort during the ride. Been riding clipless since mid 80's and other than knee pain due to improper adjustment I've had no problems. All shoes are Shimano not some really cheap piece of garbage and all have VERY stiff soles.

Any ideas?

Well, I had them pretty far forward. They are just behind the ball of my foot now but I was thinking maybe I need to move them back up, at least part way.
