Not yet!!! But I only have two rides under my belt so far with the new pedals. I had a pretty sloppy clip in at a red light this morning traffic. Language turned truck driver for a few moments.

Probably so. After 24 years of riding in toe clips and just a couple of weeks in SPDs, I just can't fathom falling over while clicked in. Release is much easier than properly tightened toe clips.

If are in type 1 or 3 (fallen) and are a masochist, try switching to Speedplays so you can repeat the falling experience.

I fell for a couple of times after switching to clipless, then a couple more times after switching to Speedplay.

True story: After twenty something years on clipless pedals I've never fallen because of clipless pedals. I imagine it's possible that in my later life years I may have slowed reaction time that makes the OP's #2 accurate, but mostly I doubt it.
