It is hard to see, but there is a tang at the anchor screw at about the 10 o'clock position if looking at the cap screw head....

It was the cable that was anchored incorrectly...

It was the cable that was anchored incorrectly (under not over) the tab. Once I fixed that, shifting is now flawless.
