Dust cap on hub

Bike rear wheel question

So I got the hubs adjusted on my wheels at bike shop but noticed the rear wheel spins for maybe 5 seconds then stops.it uses rim brakes and they were not rubbing at all.the front wheel spins freely...

on the bike fender mount hole,i use a mounting screw but it gets loose when all the way in the mount.it doesnt matter what kind of screw i use it still gets loose too.
