Learned years ago. That the issue is how the seam of the shorts is made. Cycling shorts are sewn so that their is no thread seam on the inside of the shorts. So there is no rubbing of the seam with your skin. No need to ever wear underwear with cycling shorts. Haven’t for 39 years. Just make sure to watch how thin the shorts get. As your rear starts to show.

I don't wear cycling shorts to protect me from my bike!!!! I wear them because: they are WAY more comfortable to ride in than anything else. Doesn't matter what you're sitting on. For one thing: they manage sweat WAY WAY better...when I first started riding, I rode in my street cloths. And once I started to sweat, I began to itch, and it was a drag. I wore street cloths for a year or two...I vividly recall the first day I wore cycling shorts. Feeling silly.
Went out my drive way, and by the time I was fifty yards from my house I had to ask myself: WHY THE HELL DIDN'T I DO THIS IN THE FIRST PLACE. Much better freedom of movement, nothign that interferes with how you move, way way way less problems with sweat.

When I restarted cycling two years ago I had two pairs of mountain bike shorts from Nashbar. I got these during a "false start" back into cycling around 2005. I rode in a charity bike ride in July 2019 and someone took a few pictures of me with the mountain bike shorts on my road bike. Looking at the pictures I decided I didn't like the way I look in those either. But I never felt that I was all that good riding a road bike until this year when I sheepishly set out with my first pair of spandex road shorts. I now have 6 pairs of said shorts.

we talk jackets, shorts, water proof trousers, boots etc, but i rarely see underwear mentioned.
I mostly have cotton trunks(too much information!?) but im wondering if there is a better alternative?
Merino definitely fits the bill, but its also very expensive, 1 pair of trunks is the cost of 4 or more regular trunks.
Synthetics are fine, but the smell factor is a real issue
Does anyone have any suggestions that are genuinely better than the regular cotton underwear?

FYI - They're called trunks in most stores here. Briefs, boxers and trunks are usually available.

^ right. And I do the same when tooling around the neighborhood to simply test ride a bike I'm building or repairing. The OP really didn't qualify, so I expect that he's talking about general recreational riding as opposed to task-specific utility rides like short commutes or runs to the ABC store.

Andiamo lightly padded underwear is made to be worn under regular clothing. A few in our group (including me) use them under regular shorts and have found them to be just enough to make the ride more enjoyable (combined with a decent saddle). https://andiamounderwear.com/
