check to make sure the front derialleur cable isn't hitting the crankarm. also check to see it the derailleur cage itself isn't hitting the crankarm. and that the chain isn't hitting the derailleur cage. if it's not any of those it's most likely bottom bracket related.

a few questions:
1) does it happen all the time?
2) does it only happen under pressure? (since you said it happens at 12 o'clock, i'll assume no, but still, answer!)
3) can you feel it click through your feet, or up through the frame through your booty?

4) Does it happen standing and sitting, or sitting only?

Also check that it's not something silly like a shoelace hitting the downtube or crankarm, the end of the derailleur cable hitting the crank arm, etc.

Another thing to check is for a bent tooth on the chainring. Barring all that, you might want to consider taking it into a shop to see what they say.

Its likely a pedal bearing I'm thinking, or your foot is brusing the derailleur cable.

My guess is your left hand crankarm needs a tightening or is damaged from riding loose, or the fixed cup of your bottom bracket is is loose. but best have someone with some knowledge look at the bike up close.

I was just trying to think how to ask a question with a similar problem, except my bike was new, & it's on the right side. Had already thought of pedal bearing, bottom bracket, & derailleur cage, thanks for the extra ideas.
