cog and wheel
This is probably a dumb question, but when you order a rear wheel, say a velocity b43 or a weinmann attak deep v, do they come with a cog or do you have to buy that separately?

Usually not... you might get a lockring though if the set is laced to formula.... but I would swap that cheap aluminum formula lockring for a proper machined steel dura ace one ASAP.

Your cog is slipping.
If we're talking about the wheel you're having built by Bell's Bike Shop, you'll need to purchase a cog and lockring separately.

Formula lockrings are fine, but nothing special. I had one. It was very light and felt super flimsy compared to the Dura Ace one I now have, but it did its job ok.

Miche set up is good if you change your ratio a lot, besides that, Dura Ace, EAI, All-City, EighthInch, Soma, IRO, Surly, Origin8 are just as good.

Eighth Inch and All City are the same cogs, but the Eighth Inch has the black coating on it. I think Alien also uses the same cogs, as many other companies probably do. I run an Eighth Inch and I like it, but the black coating doesn't really do anything because it wears off fairly quickly where the chain contacts the teeth.
