I saw SPDs for sale in Burma at Yangoon's Bike World Myanmar shop. The Australian owner, Phil, also runs a very nice guesthouse and bike touring service.
On cleats rotating and failing to unclip: I use Time ATACs and these cleats have some high ridges on the contact surface that bite into the sole, making the cleats impossible to rotate unless the screws have backed out a significant distance. Due to these ridges having to push into the sole, it takes quite some effort to screw in the cleats; therefore there is pressure against the screws which makes them less likely to come undone. I've been riding these for nine years and have never had a cleat come loose. Unfortunately SPD and CB cleats feature only raised bumps. SPD's bumps are much more substantial than CB's bumps.
