Handlebar bag with STI shifters

Will a Ortlieb classic 5 bag, size medium fit between shimano
shifter cables on 42mm width handlebar without mods to cables?

I'm gonna say "no." The Classic 5 is about 7" deep, plus another 2 1/2" for the holder. It's also 10" wide. Smaller bags will probably fit though.

You can measure it yourself, but I believe that without modifications, that bag will hit, bend and likely break the cables.

FWIW: I have used a slightly wider bag with similar bars with zero problems. I just gently shoved the cables down and to the side. I have done this on more than one STI equipped bike and have logged a TransAmerica (4244 miles) and a lot of other riding.

It may depend on how much slack you have in your cables and other variables of how the bike is set up, but I have never found that it put a particularly severe bend in the shift cables and consider it a complete non issue on the bikes where I have done it.
