Handlebar Stem keeps loosening

My MTB handlebar bar stem keeps coming loose from the headset no matter how much I tighten the stem. This just started happening about a week ago with no previous adjustments being made.

Now it is so bad that my simple riding and grip/pulling on the handlebars will slowly cause the stem to be pulled up from of the headset and I have to stop once on the way to work and then on the way home to reposition and re-tighten the stem.

Is there anyway to fix this short of replacing the stem and or headset?

The headset that I am using is similar to this one.

It has nothing to do with the headset. You sure have the quill bolt properly tightened and haven't stripped the bolt or wedge threads?

I know the problem is with the stem itself, the threads are not stripped nor is there any damage to stem etc. The quill bolt is tight as I can / or want to tighten it without possibly stripping it.

Maybe get a new bolt then. You and many other people may of gotten a bad batch of bolts out of the factory, the bolt could of been machined a litle to small.
