It has nothing to do with the headset. You sure have the quill bolt properly tightened and haven't stripped the bolt or wedge threads?
I know the problem is with the stem itself, the threads are not stripped nor is there any damage to stem etc. The quill bolt is tight as I can / or want to tighten it without possibly stripping it.
