Road:
Helmet: Bell Alchera
Jersey: Plain colors and cheap, but well-fitting
Shorts: Padded, preferably a thin chamois, with chamois butter on longer rides
Gloves: Temperature dependent, but always something (PI fingerless, Fox mid-weight, Swix heavy)
Shoes: Shimano something or other road shoes
If cold, add arm/leg warmers, other tights, glove liners, chemical warmers, etc...

Mountain:
Helmet: Bell Furio (yeah, I know it's a road helmet)
Jersey: Same as above
Shorts: Cargo shorts/knickers over padded road shorts
Shoes: 661 mountain shoes
Gloves: Fox full fingered, either mid-weight or thin
Glasses: cheap and shatter-proof

Commuting:
Bell Furio and whatever I happen to be wearing to work that day. The dress code is quite casual.
