I have clipless as well and I still fall over on occasion. The best was when one of the 2 screws fell out of the cleat on the bottom of the shoe. The shoe could spin 360* without disengaging (had to take the shoe off to get off the bike). Fortunately it wasn't an emergency situation. Now I use LockTite on the threads and no clipless for me while commuting.
