I need some help figuring out stem and handlebar sizing.

I know I need 1 1/8th threadless, but as for clamp and handlebar diameter, I'm seeing numbers like 25.4mm and 31.8mm. Am I correct in assuming those measurements are for only the center of the handlebars, which then taper down to whatever the standard brake lever/grips/shifter diameter at the handlebar ends?

What's the standard on size for all the handlebar accessories that I mentioned above? I seem to recall something like 7/8". TIA
