So you put the axle directly into the vice without using something like the Park AV-4? Does your vice have aluminum jaws? Even with my AV-4 I have to tighten my vise quite a bit - it seems to me like not having an axle vise -> thread damage. Quite frankly, an axle vice seems like a must-have for this job to me.
I don't have the vice very tight at all. On top of that, I clamp the vice down on the flats locknut, not the axle itself. I only use it to hold the wheel steady while disassembling one side.
