yeah i've had it happen i guess from clamping the wheel too tightly in the dropouts. the part in the picture is actually a locknut (if it's threaded), not just a spacer. it's to keep the hub cones from unscrewing on the axle. i had kind of a hard time finding a proper size replacement but ymmv. see if your bike co-op or LBS can find a match for you.
