I have an American Classic (freewheel) rear hub where the axle end caps just fall off if you remove the skewer. The end caps fit over the axle at both ends. I also have a White Industries cassette hub where there is an end-cap on the non-drive side which is held on tightly by a complicated method involving three 2mm allen set screws. With that one the end cap fits inside the axle.
