I actually took the whole thing apart, cleaned it, and, true to Campy quality, it's nice and smooth now. So, I will keep it. It Is a Campy integrated headset. The tiny bearings fooled me, but I'm used to the classic Campy Record headsets with nicer large bearings. These little BB's do the trick though, once cleaned and greased properly.
OK, that answers my first question. Q: Did the headset really have to be replaced? A: No.

All of the Shimano bolt-on cable housing stops (the ones that bolt over downtube shifter bosses) and the braze-on (really weld-on) stops on both of my Litespeed frames are threaded M5x.8 mm. Also, the adjuster barrel in a Shimano rear derailleur is likewise M5x.8 and, as best I can tell, the adjusting barrel on a Campy rear derailleur is the same. Smart money says M5x.8 mm.
