Mavic 571, 840, and 821...why won't they index?

I've been playing with the Mavic bits I've acquired and can't seem to get the 821 shifters to properly index on the 840 over the 571/2 hubs (8 Speed). It seems as though the levers are actually seven speed (six distinct clicks), which doesn't jive with what I've been able to research.

I've read about the routing options with the pivoting plastic cam near the barrel adjuster but haven't been able to find an illustrated example of how to route the cable for 8 speed operation: through the hole or around the edge. Itarweb research landed me in the land old message threads that linked to 404 not found illustrations.

So, do the 821 levers do 8 speed?

If so, how do I route the cable: with the cam parallel to the body (and thus around) the cam or with the cam perpendicular to the body (and thus through the cam).

Does anyone have the original documentation for the 821 and 840? I'd RTFB if I had the "B"...

Plan B is to simply abandon the 821 levers and just use some 8 speed RX-100s I had in the parts box.
