One possibility for medialis pain (in the muscle, not near the knee) is kinking of the external iliac artery. When effort is intense in the flexed position, the artery can be kinked, temporarily (i think) reducing blood flow to the inner areas, causing an almost cramp-type pain, especially in the medialis. It's not uncommon for cyclists.
The impeded flow doesn't show up on normal 'pulse' tests, where the regular doc detects the pulse around the ankles.
